What documents are required for Indians to get the entry permit into Bhutan? Is an organized tour compulsory?

We are three indians planning to go to Bhutan next month. I gather from various websites that Indians don't need a visa, but only a permit to tour the country. I want to know what documents we need to carry for that. Also, I have read that for foreign tourists it's compulsory to subscribe to a package tour. Is it compulsory for Indians also? If not, can someone help with respect to places to see, stay, eat, etc?
